1	Title: Enter security question and answer
2	Instruction text: Now choose a security question and answer to retrieve your username or password if you forget them later on. This is recommended but not mandatory.
3	How does it work? If you have forgotten your username or password, go to 'Forgot username or password' in the login screen. Answer the security question of your choice. If the answer to the security question is correct, we will e-mail your username and password to the e-mail address you provided to us, which is: <doorgegeven e-mailadres>.
4a	Choice list: Security question Click on the drop-down list and choose one of the following options: What is the initial password ion the invitation letter from the Tax Office? What is my customer number with my energy supplier? What is the card number of my bank card? What is the expiry date of my driving licence (dd-mm-yyyy)? You must make a choice to continue.
4b	In the text box, formulate the answer to the question you have chosen. Try to come up with an answer that you can exactly reproduce.
5	Text box: Answer. The answer to your chosen question may be up to 100 characters long.
6	Button: Approve
7	Button: Cancel
