This form is intended for a refund of VAT paid on the supply of goods and VAT and excise duty paid on the purchase of motor fuels by (staff members of) embassies, consulates and civil society organizations in the Netherlands.

This form can be completed on your computer. The fully completed form can be printed and signed at question 5 of the form. Placing a digital signature is also allowed.
