If you were provided with a 'Cash disclosure declaration' by Dutch Customs and need to complete (another) extra sheet, you can do so using the 2nd form.
If you were provided with a 'Reporting right - disclosure declaration Freight, postal, parcel and courier shipments' by Dutch Customs and need to complete (another) extra sheet, you can do so using the 4th form.
Dutch Customs will provide you with the 1st and 3rd form. If you need an additional copy, you can re-download the form.
Instructions for filling in the form
You need Adobe Reader 9.0 or higher.
- fill in the form
- print the form
- sign the form
- send the form to the address mentioned on the form
Problems with completing the form?
It is best if you complete the form on a computer. We cannot guarantee a proper functioning on a tablet or smart phone.
If you are unable to complete the form on your computer, you should first save the form. You can do so by right-clicking on the link of the form and selecting 'Save target' (this term may differ per browser).
