Requesting tax forms

Read below to find out which tax form should you use:

• 2009 Tax return Form C
• 2009 Tax return Form Tc
• 2009 Tax return Form M

There are different application procedures for requesting C, Tc and M forms.

Requesting a 2009 C Form

You can apply for a C-Form in writing or calling: Tax and Customs Administration/Limburg/Department of International Issues.

If you write a letter, please state your name, address and tax and national insurance number (sofinummer).

Requesting a 2009 Tc Form

You can order the 2009 Tc Form (only in Dutch).

Requesting a 2009 M Form

You can apply in writing for a 2009 M Form with the tax office that is competent in your case. Please state your name, address and tax and national insurance number (sofinummer) in your letter.
